Alkaline Phosphatase (ALP): Normal Range and What High ALP Means
Alkaline phosphatase comes mainly from two places — the bile ducts of the liver, and bone. That dual origin is the whole difficulty in reading it: a high ALP asks a question (liver or bone?) that the number itself cannot answer.

What a high Alkaline Phosphatase (ALP) means
A high ALP alongside other raised liver values usually points at the bile ducts rather than the liver cells themselves. Raised on its own, with normal liver values, it more often points at bone. A doctor may add a further test to separate the two.
Common causes:
- Blocked or inflamed bile ducts
- Gallstones
- Fatty liver and other liver conditions
- Bone conditions, including vitamin D deficiency
- Healing fractures
- Normal growth in children; pregnancy
What a low Alkaline Phosphatase (ALP) means
A low ALP is uncommon and is usually not significant on its own; it is occasionally seen with malnutrition or certain rare conditions.

When to see a doctor
See a doctor if ALP is clearly above the range, particularly with itching, yellow eyes, pale stools, dark urine, or bone and joint pain. A mildly raised ALP in a growing teenager or in pregnancy is usually expected rather than alarming.

What causes high alkaline phosphatase?
Most commonly a problem with bile flow in the liver, or something affecting bone — including vitamin D deficiency, which is widespread in India. Growth and pregnancy raise it normally.
Is a high ALP always a liver problem?
No. If the other liver values are normal, bone is at least as likely a source. Doctors distinguish them using the rest of the liver panel and, where needed, a further test such as GGT.
Can vitamin D deficiency raise ALP?
It can. Low vitamin D affects bone turnover, which is one of the more common and correctable reasons for a mildly raised ALP.
